- Results From Two Large, Multinational, Practice-Based Observational Studies in Blood Pressure MILAN, Italy, June 18 /Xinhua-PRNewswire/ -- New data announced today at the Annual European Society of Hypertension (ESH) show that measuring blood pressure in a doctor's office alone does not provide an accurate picture of a patient's blood pressure control. Treatments utilised only on this basis are likely to leave many people at increased risk of heart attack, stroke or even cardiovascular death. According to a further study also presented today, this "unnecessary" additional risk could be avoided by choosing a treatment regimen like once daily telmisartan, which provides smooth blood pressure control over a full 24 hour period, including the risky morning hours. Results from the SURGE hypertension study, an observational study of blood pressure control outside of a clinical setting, demonstrate that:(1,2) - More than 60% of patients do not have controlled (<135/85mmHg, ESH/ESC) blood pressure in the morning when measured by home blood pressure monitoring (HBPM). - Again, more than 60% of patients are not able to achieve control of blood pressure peaks over a 24-hour period, when measured by ambulatory blood pressure (ABPM), which monitors control over the full 24-hour period. Professor Gianfranco Parati, University of Milano-Bicocca, Italy, Chair of the SURGE Steering Committee commented, "The SURGE study is noteworthy because it illustrates that in a real-life setting, despite receiving treatment, a large proportion of patients still have poorly controlled hypertension, particularly in their morning hours. It also highlights the challenges physicians face in getting an accurate picture of blood pressure as HBPM and, even more so, ABPM can not be implemented routinely in every patient in daily practice. What we really need are reliable therapies that provide consistent control over a full 24 hour period." SURGE-2, an 8-week, open-label prospective, practice-based study, assessed the effects of the angiotensin II receptor blocker (ARB) telmisartan in >26,000 patients with hypertension (office blood pressure >140/90mmHg). Blood pressure control was measured using either HBPM (n=25,882) or ABPM (n= 863). The results(3,4) from this observational study, the largest practise-based study ever conducted in hypertension using this methodology, demonstrate that telmisartan provides powerful blood pressure control over a full 24-hour period, including the risky early morning hours(3,4): - The mean morning ABP was significantly reduced by telmisartan alone and in combination with HCTZ by -8.2/-4.9 mmHg (p<0.001), almost doubling the number of patients with controlled blood pressure from 36.5% to 64.4% (<135/85mmHg, ESH/ESC) after 8 weeks of treatment. - The 24-hour ABP was reduced by telmisartan alone and in combination with HCTZ by -7.85/-4.7 mmHg (p<0.001). Professor Parati comments, "The results from SURGE-2 are particularly encouraging. They demonstrate that, unlike many other antihypertensive treatments, in telmisartan we have a treatment option that does provide smooth blood pressure control from morning to morning. This is expected to lead to prevention of heart attacks, strokes and even cardiovascular death." The SURGE studies (SURGE and SURGE-2) are part of an extensive ongoing clinical trial programme, with observational as well as clinical studies investigating the outstanding effects of telmisartan compared with other treatments for hypertension, including other available ARBs. These trials investigate the effects of telmisartan in providing powerful blood pressure reduction from morning to morning, potential organ-protective effects and cardiometabolic protective properties. Telmisartan has a longer duration of action than all other members of the ARB class; it takes approximately 24 hours for half the dose of telmisartan to be eliminated from the body compared to five to 15 hours for other ARBs(5,6). Clinical trials have already shown that telmisartan provides powerful and consistent blood pressure reduction over a full 24 hour period(5-8). The results of the SURGE studies in a real life setting further support the hypothesis that, due to its unique pharmacological profile, telmisartan could provide a solution for the ongoing problem of risky blood pressure surges in the morning. Ultimately, telmisartan may give doctors an effective option to protect their hypertensive patients from cardio- and cerebrovascular events 24 hours a day. MILAN, Italy, June 18 /Xinhua-PRNewswire/ -- Today AMADEO, one of the first studies to compare the protective potential of two angiotensin receptor blockers in hypertensive patients with diabetic nephropathy, was presented at the European Society of Hypertension, Milan. The results from this study show that telmisartan reduces proteinuria to a significantly greater extent than losartan.(1) Commenting on the results, Prof. Ellen Burgess, Foothills Hospital in Calgary, Canada said "The AMADEO results are encouraging for an increasing number of patients with type 2 diabetes because they suggest that telmisartan could improve renoprotection. It is particularly interesting that the observed effect was seen despite the study being controlled for blood pressure." She continued, "telmisartan has already shown superior blood pressure lowering compared to losartan and, in AMADEO, patients were allowed to take other medication, if needed, to ensure similar blood pressure in both treatment groups. This suggests that the protective benefits seen with telmisartan here are an additional attribute beyond its established blood pressure lowering effects." Diabetic nephropathy is a kidney disease that occurs in approximately one third of patients with diabetes mellitus(2). These study results could, therefore, have a positive impact on millions of type 2 diabetes patients as well as healthcare systems worldwide. The prevalence of diabetes is projected to increase at an alarming rate from 171 million in 2000 to 366 million by 2030(3), with the upsurge in obesity closely linked to increased type 2 diabetes. Over time, diabetic nephropathy can lead to end-stage renal disease, a serious condition that needs dialysis and increased medical care and resources. End-stage renal disease has tripled in prevalence over the past two decades(4) and has huge associated healthcare costs, predicted to be US$28billion by 2010 in the US alone(5). AMADEO, a randomized, double-blind, forced-titration, parallel-group, multicentre study, included 860 hypertensive patients (>130/80mmHg) with type 2 diabetes and overt nephropathy from 124 centres in 10 countries. Patients were randomized to receive treatment with either telmisartan 80mg or losartan 100mg. To ensure blood pressure control in the two patient groups other non- ARB treatments (hydrochlorothiazide or calcium channel blocker) were added, if needed. After one year's treatment, telmisartan was significantly more effective than losartan in reducing the amount of protein excreted in the urine. The primary end point of the study was reduced by 29% with telmisartan vs. 20% with losartan; p=0.0284(1). Telmisartan was superior to losartan on the primary endpoint, a change from baseline after 12 months (log transformed Urinary Protein creatinine ratio) of 0.71 (95% CI; 0.66, 0.77) vs. 0.80 (95% CI; 0.74,0.87) for losartan; p=0.0284.(1) No significant difference in blood pressure control or number of adverse events was observed between the two treatments groups(1). Proteinuria (high levels of protein in the urine) is a very important signal for disease severity in diabetic nephropathy and is also considered a relevant cardiovascular risk factor. Renal outcomes trials have shown that reductions of >30% at six months are strongly linked to slowed progression to end-stage kidney disease and reduced cardiovascular events.(6) AMADEO successfully concludes the series of PROTECTION studies which are part of an extensive ongoing trial programme, including clinical and observational studies, investigating the outstanding effects of telmisartan compared with other treatments for hypertension, including other available ARBs. The trials established the effects of telmisartan in providing powerful blood pressure reductions from morning to morning as well as organ-protective effects. Telmisartan has a longer duration of action than all other members of the ARB class; it takes approximately 24 hours for half the dose of telmisartan to be eliminated from the body compared to five to 15 hours for other ARBs.(7,8) Telmisartan is 99.5% bound to serum protein and is excreted almost entirely via non-renal pathways. Clinical trials have shown that telmisartan provides powerful and consistent blood pressure reduction over a full 24 hour period.(7-10) About Telmisartan (Micardis(R)/Kinzal(R)/Pritor(R)) Telmisartan is a member of the angiotensin II receptor blocker (ARB) class and is being investigated in the most ambitious and far-reaching research programme ever conducted with an ARB. DRESDEN, Germany, June 18 /Xinhua-PRNewswire/ -- The California-based solar module manufacturer Signet Solar broke ground today on its main European factory. The facility, located near the eastern German city of Dresden, will serve as a production line as well as headquarters for research and development. The start-up will invest EURO50 million into the facility that should begin operations mid-way through the year 2008. The company expects then to have 130 employees at the location. On the site, the company will design and produce solar (photovoltaic) modules. These devices convert the sun's rays into electricity. Signet Solar hopes that its products can help achieve the goal of "grid parity" for solar energy within the next 5 years. "Grid parity" means that electricity derived from solar energy sources will cost the same as energy originating from conventional sources such as fossil fuels. The company's investment is another success story for the eastern German region known as "Silicon Saxony" because of the concentration of solar and semi-conductor firms in the area. Currently there are 250 companies, with a total of 17,000 employees engaged in the semi-conductor, electronics, and micro system industries in the Saxony area. 今までオーディオPC(PCオーディオとも言う)は大容量のハードディスク・
ドライブ(HDD)を搭載する事で、CDから音楽データを取り込んでいました。
この大容量のハードディスク・ドライブ(HDD)は、便利でしたが、
下記のような問題がありました。
1)高速回転するため振動や騒音が大きく、オーディオに向かない
2)アクチュエーター(信号を読み取るためのアーム状の機構)
を正しく動作させるために強力なサーボをかける必要があり、
これに伴うサーボ電流と磁気の急激な変動が、非常に大きな
電磁ノイズとなる
3)PCとしての故障原因の第一位がハードディスク・ドライブ(HDD)
であり、故障した際にはすべてのデータを失ってしまう
今回オーディオPC本体をCF Memoryによる駆動とし、大容量のハードディ
スク・ドライブ(HDD)を、LAN上やIEEE1394/USB 2.0接続に変更する
